Faith Family Medical Center is a local nonprofit clinic that exists to offer hope and healing to the working uninsured and other under-served in the greater Nashville area. So many in our region are working hard to make ends meet, and simply cannot afford the high-quality medical care they need. At Faith Family, we want to be more than just a primary care clinic – we want to be your healthcare home. That is why we offer more to our patients than what is offered in a primary care setting, including behavioral health and a wellness program. Patients pay on a sliding scale based on their income, with the average patient paying an average of $29 per visit, including lab work.
All patients have free access to a registered dietitian through our Journey to Wellness program, which empowers them to get healthy through exercise and nutrition. Patients have opportunities to enjoy group classes, one-on-one sessions, and events throughout the year, offering the support that motivates them to lead healthier lives.
To supplement our wellness program, we offer monthly chronic disease days for patients with Diabetes or Hypertension. On these days, the clinic is reserved for those patients to have all their yearly appointments done at once. We have partnerships with more than 160 specialists across the region who see our patients at a free or reduced rate, offering comprehensive preventative and specialized care. Since we are a nonprofit, we utilize these partnerships and other resources to keep patient costs low.
